Biggest Caddie/Golfer rules blunders.

The Michelle Wie thread got me thinking -- I guess her caddie is supposed to remind her to stay in the scoring tent until her scorecard is signed. Sure, the buck stops with her, but the caddie's there to help.

What are some other caddie/golfer blunders like that? I remember Ian Woosnam playing in the last (or next to last) group on Sunday at the 2001 British Open . . . He nearly made a hole-in-one on the first hole . . . but he had 15 clubs in the bag.
 
In 1968, Roberto DiVincenzo signed an incorrect scorecard after the final round and was DQed as the winner of the Masters.

I defy anyone to trump that fuckup.
 
DiVincenzo may have won, but the error kept him out of a playoff. It didn't take away the win for sure.

Similar thing happened at Q school in 1999. Jaxon Brigman made the 412 he needed to get his Tour card. His partner, though, wrote him down for a 4 instead of a 3 on a hole. So the incorrect score stood and he officially recorded a 413, one shot away from the Tour. He still hasn't made the tour, I don't think.
 
How about the Euro player whose caddy forgot to take a club that the player was experimenting with out of the bag. Cost the guy stroakes (maybe 2?).

Can't recall his name. The player didn't fire the caddy, but did a tournament or two later after another blunder.

The guy that put the towel down so that he could hit off his knees was Craig Stadler. He was penalized for "improving his stance".

As far as any of the mentioned "blunders", only one can be classified as a mistake by the caddy (Woosnam incident). Michelle Wie is the only person to blame for her mistake. She is responsible for knowing the rules, not her caddy.
 
I read once that, in the 2000 U.S. Open, after knocking his first ball in the ocean (infamous "****" shot) on 18, Tiger had only one ball left. If he would have lost it he would have been DQd.
 
JohnnyM's link has a story that pisses me off.

The Kite story is good. The teenager watching his opponent commit a penalty and waiting until after the stroke is made to point it out is ********.
